When you request an appointment, our team will review your information, verify benefits and scheduling availability, and send a confirmation text once your appointment has been approved.
If you do not see a time that works for your schedule, please submit the appointment request that best fits your needs and our team will contact you regarding additional options.
We are in-network with Blue Shield and Cigna PPO.
Self-pay patients may have access to earlier availability and additional scheduling flexibility. You may request the earliest appointment that works for your schedule, and our team will follow up to confirm availability and review self-pay options with you.
If you need prompt care or flexibility, you may find appointments sooner than our insurance-based scheduling. Please text us if you have specific needs.
To request an appointment with Dr. Art Van Leuven, please text our office directly. Dr. Van Leuven works exclusively with private-pay patients and his schedule is not visible online.
