AI in Physical Therapy vs. Automation: What We’re Actually Using in a PT Clinic Today

Artificial intelligence is suddenly everywhere in healthcare.

There are AI documentation tools, AI scheduling systems, AI receptionists, AI-assisted billing platforms, movement-analysis tools, and plenty of products that simply add “AI” to their marketing.

But what does AI in physical therapy actually look like right now?

At SB Physio, we’ve been adopting new technology for years, and more recently that has included artificial intelligence. Some of it has been extremely useful. Some hasn't been ready for what we need it to do. And quite a bit of technology that gets lumped into the AI conversation isn't really AI at all—it's automation.

From the perspective of a physical therapy clinic actually using these tools every day, that distinction is worth understanding.

AI vs. Automation in Physical Therapy

Automation and artificial intelligence aren't the same thing.

Automation generally follows a predetermined process:

When this happens → do this.

If you schedule an appointment and immediately receive a confirmation text, that's automation.

If an appointment reminder automatically goes out the day before your visit, that's automation too.

Artificial intelligence goes a step further. Instead of simply following a predefined rule, AI can analyze information, recognize patterns, work with language, generate content, or make predictions based on the information it's given.

The line isn't always perfectly clear anymore because modern healthcare software increasingly combines the two.

And from our perspective, the label isn't actually the important part.

The important question is whether the technology makes it easier for our team to take care of patients.

Automation in Physical Therapy Is Already Everywhere

Some of the most useful technology in our clinic isn't particularly futuristic.

Online scheduling is a good example.

Rather than requiring every patient to call our office during business hours, patients can see available appointments and schedule online. Automated systems can then handle confirmations and reminders.

Text messaging has also changed how we communicate with patients.

A patient doesn't necessarily need to call, wait on hold, leave a voicemail, and wait for someone to call back just to tell us they're running late or ask a simple scheduling question. They can text our office and communicate directly with our team.

Digital intake forms, appointment reminders, scheduling links, automated follow-ups, and other workflows can eliminate a surprising amount of repetitive administrative work.

None of that necessarily requires artificial intelligence.

But it matters.

Every routine task that technology can reliably handle is one less task competing for our staff's attention.

Where We’re Actually Using AI in Physical Therapy

One of the clearest uses of AI in physical therapy today is clinical documentation.

Our physical therapists use an AI-assisted documentation system that can listen during a patient encounter and help create the clinical note.

This doesn't mean AI is deciding what is wrong with the patient or determining the treatment.

The physical therapist still performs the evaluation, makes clinical decisions, determines the plan of care, provides treatment, reviews the documentation, and remains responsible for the final note.

What AI can do is help with the administrative burden surrounding that process.

It can organize information from the encounter, generate portions of the documentation, and assist the clinician with completeness and compliance.

That can be a meaningful difference during a busy clinical day.

We don't want our physical therapists spending a patient visit staring at a computer trying to type everything that is happening.

If AI documentation allows the therapist to spend more of that time interacting with the person sitting in front of them, that's a useful application of the technology.

AI Is Also Moving Into Physical Therapy Billing

Another area patients rarely see is the amount of work that happens after a physical therapy visit.

Claims have to be submitted and processed. Problems need to be identified. Denials need to be reviewed. Documentation and billing requirements need to be checked.

AI is beginning to help with this work as well.

Instead of requiring a person to manually review every piece of information, software can help identify potential claim problems, analyze denials, flag items that need attention, and help staff prioritize where human intervention is actually necessary.

Again, the goal isn't removing people entirely.

It's allowing people to spend their time on the situations where they actually add value.

Can AI Help Reduce Missed Physical Therapy Appointments?

Scheduling is another area where AI and automation in physical therapy are starting to overlap.

Basic appointment reminders have existed for years.

Newer systems can potentially do much more: identify scheduling patterns, recognize patients who may be at greater risk of missing an appointment, improve follow-up, help fill cancellations, and determine when additional communication may be useful.

We're experimenting with some of these newer capabilities now.

A missed appointment isn't just an empty spot on a schedule. It can interrupt someone's plan of care.

At the same time, having staff manually chase every appointment, cancellation, and scheduling issue takes time away from other patients.

If technology can help solve both problems, that's valuable.

What About an AI Receptionist?

This is one area where our experience has been much more mixed.

We've experimented with AI receptionists.

The potential is obvious.

An AI receptionist could theoretically answer after-hours calls, help when several patients contact the clinic simultaneously, answer routine questions, or assist with scheduling when our front desk is busy.

We would still like technology to help us solve some of those problems.

But so far, nothing we've tried has been as good as our actual front-desk team.

Physical therapy communication gets complicated surprisingly quickly.

A patient might not know exactly what type of appointment they need. There may be an insurance question. Someone may have just had surgery. Another person may need to change several appointments. Someone else may simply be trying to explain a situation that doesn't fit neatly into a predefined workflow.

Good front-office staff understand context.

They ask follow-up questions. They recognize when something sounds unusual. They know when to involve a physical therapist. And they understand the individual needs of our clinic and our patients.

AI is getting better quickly.

But this is also a good example of why we don't believe every job should be automated simply because technology exists that claims it can do it.

The Next Step: AI and Objective Physical Therapy Data

This may ultimately be one of the more interesting applications of AI in physical therapy.

Physical therapists have access to increasingly sophisticated technology for objectively measuring human movement.

We can measure things such as strength, range of motion, balance, movement quality, side-to-side differences, and changes over time.

The first generation of this technology primarily helped us collect better data.

The next step is helping us understand it.

As AI begins to get layered into objective testing systems, it may become possible to recognize patterns across much larger amounts of information than a clinician could realistically process manually.

That doesn't mean an algorithm replaces the physical therapist's examination.

The more interesting possibility is that it gives the physical therapist another tool.

The technology collects information. AI helps identify patterns or changes within that information. The clinician then combines that with the patient's symptoms, history, goals, examination, and response to treatment.

We're still early in that process, but this is an area we're watching closely.

So Is AI Going to Replace Physical Therapists?

Not ours.

At SB Physio, our model is built around one-on-one care and direct interaction between the patient and physical therapist. That's where we believe the value of physical therapy is, and AI doesn't change that.

In fact, much of the reason we're interested in AI and automation is to protect that time. If technology can take documentation, administrative work, scheduling tasks, or information processing off a clinician's plate, we'd rather have that time and attention available for the patient.

For the profession more broadly, AI will almost certainly change what some parts of a physical therapist's job look like.

Right now, though, the most useful applications of AI in physical therapy aren't actually doing physical therapy. They're helping with the enormous amount of work that surrounds it.

AI can help write a note, review information, flag a billing problem, identify patterns, or help determine when something needs a person's attention. Automation can schedule an appointment, send a reminder, deliver an intake form, or make communication easier.

Even as these systems become more sophisticated, there is a significant difference between processing information and actually managing a patient.

Physical therapy involves clinical reasoning, physical examination, movement analysis, communication, and constantly adapting to what happens during an interaction. A patient rarely presents as a clean set of data points. What they say, how they move, what changes when you try something, and what actually matters to them all influence what happens next.

AI will almost certainly become better at assisting with parts of that process. It may give clinicians better information, recognize patterns we might otherwise miss, and eliminate more of the administrative work surrounding care.

That could change what the job of a physical therapist looks like.

But that's different from replacing the physical therapist.

The more realistic near-term future is probably a physical therapist using AI rather than an AI physical therapist.

Our Approach to AI and Automation at SB Physio

Whether something is called AI, automation, machine learning, or simply better software isn't particularly important to us.

Our standard is simpler:

Does this technology take away work that was costing us patient care, patient interaction, or service time?

If it does, we're interested.

We expect these tools to become much more sophisticated, and we'll continue experimenting with them. The goal remains the same: use technology behind the scenes so we can put more of our time, attention, and resources where they have the most value—with the patient.
